- For minors (ages 17 or younger) parents need to fill out this form.
- The musician's information goes on the first page and the contact details requested on the second page is for the parent.
- If you're an adult (18+) your contact info will be automatically filled in on the second page.
We do ask for birthdays, because we like birthdays, but if you don't want to put your birth year in please just put your birth date as 2023 or 2024. Thanks!
